The following question is required, in order to ensure the safety of all participants, staff and volunteers in the Women In The Field Art and Music Festival.

a. any offense committed before your 18th birthday.
b. a first conviction for any of the following misdemeanors: drunkenness, simple assault, speeding, minor traffic violation, affray, or disturbance of the peace.
c. a misdemeanor conviction when the date of conviction or the ending date of any resulting period of incarceration/probation/parole was 10 or more years prior to the date of this application and you have not been convicted of any offense in the last 10 years. If you have been so convicted, you must report all offense convictions that occurred before and during the 10-year period. List offenses in section below.
d. an arrest detention or disposition where there was no conviction.
e. an applicant with a sealed record on file with the Commissioner of Probation may answer "No" relative to prior arrests, criminal court appearances or convictions.
